 | Cardiac Rehabilitation is a dynamic concept and action program where the various skills of the rehabilitation team consisting of a physician, nurse, and other trained personnel are integrated as a single force to assist the patient in reaching his maximum physical, emotional, social and vocational potential. Cardiac Rehab is initiated by a physician.
A functional assessment and evaluation are performed by a qualified professional. A treatment plan is developed for each individual patient and, if appropriate, patient and family are involved. Measurable goals are established and a treatment program is designed to achieve these goals. Results of treatment and progress are assessed on at timely basis. Goals are revised as needed. The patient’s progress and result of treatment are documented in the medical record.
