 | Health Information Management is a vital part of Lawrence County Memorial Hospital’s functions to be able to provide qualified prompt care to our community. The Health Information Management Department is the main source for your medical records and protected health information.
Health Information Management are liable for patient records to be kept on file by all outpatient, emergency room and in-hospital stay services provided by hospital staff and physicians. This allows physician access to review patient medical record – history for assistance in providing necessary quality care.
Health Information Management professionals are responsible for the development and administration of healthcare data collection and reporting systems that ensure the quality, integrity, availability and preservation of data in support of patient safety, privacy, confidentiality and security.
Services provided in our area include release of information and coding of records for submission to insurance companies for billing purposes so that Lawrence County Memorial Hospital may continue to be a service to the community. Specific forms are necessary for any release of information as required by Lawrence County Memorial Hospital policy and federal Health Information Portability and Accountability Act (HIPAA) regulations. Patients will be required to present personal identification for release of such information. A charge for the cost of copying and sending the records may also be applicable.
The Health Information Management Department of the hospital is located on the first
floor of the hospital. The department is open from 7:00 a.m. to 4:30 p.m., Monday through Friday. |  |
