Gifts of $20,000 - $99,999

Gifts of $5,000 - $19,999
- Partners
- Investors
- Friends
Please make checks payable to E & D Foundation for LCMH. Donations are accepted at the hospital or can
be mailed to: E&D Foundation for LCMH, 2200 West State Street, Lawrenceville, IL 62439
All Contributions to the Foundation are deemed charitable under section 501(c)3 of the Internal Revenue
Code and are tax deductable!
