Providing referrals to Home Health agencies, long term care facilities, outpatient treatments and services, skilled therapies and advanced directives (DNR forms and Power of Attorney).
The social service department works with other area community agencies on an individualized basis creating a network of trust and quality care.
Lawrence County Memorial Hospital Swing Bed Program:
- Serving the Medicare population
- Who requires a skilled need
- providing quality extended care for those patients recovering from a acute illness or condition
- Allows for personal, individualized professional care in a hospital environment without admission to a nursing home
Examples of skilled needs include:
Physical Therapy, Intravenous Therapy and wound care
