Medical Records

There are several ways to get a copy of your medical record. Start the process by: 

• Downloading the release form, or
• Stopping by the Health Information Management Department on the first floor of the hospital (Monday - Friday, 7:30 AM – 4 PM), or
• Call us at 618-943-7226

Please note that in compliance with state and federal laws, all requests must be submitted in writing, completely filled out and signed.
On the form you will indicate how we should release the records - if you will pick up the records or if we send them to you or your doctor. 
Please allow 5-10 days for requests to be processed. 

 

Completed and signed request forms can be mailed, faxed or delivered to:

Birth Certificates
